We empower Allied Health professionals, hospital executives, discharge planners, and support services with the knowledge, tools, and networks needed to deliver safe, timely, and person-centred outcomes.

Aged Care Transitions – supporting older adults over 65 who are ready to move from hospital to nursing or residential care.

Through targeted masterclasses, workshops, events, and practical resources, Impact Housing fosters collaboration between hospitals, allied health teams, SIL providers, disability services, and housing organisations.
